If you say Leomon's name around a hardcore Digimon fan, they're either going to start tearing up or start laughing as a defense mechanism. This Digimon has repeatedly met his end in a dramatic fashion, to the point where his presence in a given series signals that a tragedy is around the corner. Technically, all of these Leomons are slightly different entities, but since they present as the same guy, it counts.
In the first series, Leomon traumatized the Digidestined when MetalEtemon took him out in front of their eyes. In Tamers, his partner Jeri is made so miserable by his demise that she's easily corrupted by the D-Reaper. Then there's SaberLeomon, whose data is corrupted so that he can't come back to life aferward... the list goes on.

In the world of Steins;Gate, time travel might be possible, but that doesn't mean that everyone can be saved. Mayuri Shiina is a sweet, innocent girl who is one of Rintaro Okabe's childhood friends. Though he tries to use time travel to save her, the Attractor Field Convergence results in her losing her life over and over again.
Mayuri has a heart attack, gets shot in the head, gets hit by a car, gets hit by a train, gets turned into gel while being sent back in time - and that's not even a full list.

(#17) Miyubi's Recurring Deaths Become A Running Gag In 'Seton Academy: Join the Pack!'
At Seton Academy, you'll find a plethora of magical anthropomorphized animals who defy the laws of nature. So naturally, it's not an uncommon occurrence every time Miyubi Shishio (AKA Sloth Girl) dies in every episode. Because she's a sloth, she easily gets overheated and tired from the slightest movements. Things like walking too much or carrying a heavy tool are enough to kill her. She has even died because of the wind. This happens so often that the series made her recurring deaths into a running gag.

(#5) Kazuma Satou's Existence Stops Making Sense In 'KonoSuba'
The first time Kazuma Satou passes away, it's under embarrassing circumstances. He tries to save a girl from being hit by what he thinks is a truck, but which turns out to be a tractor that's barely moving. No one is hurt, but Kazuma is so shocked by the experience that the emotional upheaval ends his life.
Afterward, Kazuma is reincarnated into a fantasy world. There, he loses his life multiple times, but is always revived thanks to the magic around him.

Hyatt has kicked the bucket so many times that somebody actually made a death counter for her, and it topped out at 51 times. This usually happens due to her sickly nature - she's constantly passing out and coughing up blood. It's unclear what exactly is wrong with her - the manga implies the Il Palazzo implanted something gnarly in her body, while the anime indicates that her martian heritage makes it difficult for her to breathe Earth's atmosphere. Whatever the reason, Hyatt loses her life multiple times per episode.

Everybody in the world of Angel Beats has lost their lives at least once - that's why they're hanging out in what is basically purgatory.
